- How does the Physician Office Visit coverage work?
- Each time you, your spouse or your eligible children visit a doctor's office for treatment as a result of an accidental bodily injury, the benefit amount you selected will be paid. Treatment must occur within 30 days of the accident causing the injury. Only one benefit is payable for each accident and a maximum of 12 visits are payable per calendar year for all covered persons combined.
- How does the Accidental Death Benefit work?
- For this benefit to be paid, your death must have resulted directly and independently from accidental bodily injury. The accident causing such injury must occur after the Effective Date of Coverage.
- How does the lump sum Accident Hospital Indemnity Benefit work?
- Each time you, your spouse or your eligible children are confined in the hospital for more than 60 days as a result of accidental bodily injury, the benefit amount you selected will be paid. The confinement must begin within 90 days of the accident causing the injury.
- How does the Emergency Room Benefit work?
- Each time you, your spouse or your eligible children are required to receive emergency room treatment due to a covered accidental injury, the benefit amount you selected will be paid. Only one visit per calendar year per covered person is payable.
- What does the Dental Benefit cover?
- This Plan provides coverage for everything from a basic cleaning to full-scale orthodontics. You simply choose a physician from a network of over 12,000 dentists and specialists nationwide. There are no exclusions and no maximums. Complete details will be included in your certificate of insurance.
- What does the Vision Benefit cover?
- You will be covered for normal optometry prescriptions, as well as LASIK surgery. This includes exams, glass or plastic lenses, bifocals or trifocals, and all tints and coatings. Complete details will be included in your certificate of insurance.
- Can I save on generic as well as brand-name prescriptions?
- Absolutely. There are 40,000 drug stores that are ready to fill your discounted prescriptions, both generic and brand-name.
- Can I receive these savings benefits for treatment by any physician?
- Treatment must be provided by one of the over 350,000 physicians that are part of the Health Advantage Plan network; one of the largest preferred-provider networks in the United States. If you have health insurance, these discounts can help with expenses that may not covered by your current policy, such as routine office visits and cosmetic surgery. If you have no health insurance, this Plan can help save you money on visits to a participating physician. And new physicians are being added all the time.
- Can you explain the Patient Advocacy program?
- The Patient Advocacy program is designed to provide you with personalized attention. A provider team will serve as your Patient Advocate, guiding you through the process of:
- Finding primary care physicians and assisting in making appointments;
- Certification of discounts from the providers in the Plan;
- Assisting in finding hard-to-reach specialists and critical illness physicians and medical institutions;
- Identifying specialist physicians and medical institutions;
- Solving billing and related administrative issues;
- Accessing community resources;
- Finding senior care services that fall outside traditional health insurance coverage. It’s an invaluable tool that many insurance plans don’t provide.
- What is Organized Wisdom?
- Simply put, it's a members-only website with fully screened and high-value medical, health and wellness information.
- What type of hearing services can I receive discounts on?
- This Plan provides free hearing aid maintenance, as well as significant savings on hearing exams, and 10% off on hearing products.
- Can I save on chiropractic services?
- Yes. A network of chiropractors will provide a free initial consultation. Then you can receive 50% off diagnostic services and necessary x-rays, and 30% off all other services. (Note: discounts or x-rays do not apply in Colorado.)
- What's the 24-Hour NurseLine benefit?
- The NurseLine benefit is a 24-hour-a-day, 365-day-a-year telephone nurse advice service that is available to members and their families. NurseLine puts you in touch with a registered nurse or answers on healthcare questions. NurseLine also offers access to an audio health information library in both English and Spanish.
- What if I have other insurance coverage?
- This coverage was designed to supplement any coverage you already may have. It pays regardless of any existing coverage.
